This questionnaire is to be given to the couple to fill out individually at home and brought back at a later appointment.

1.  Give the first names of your parents and siblings, their ages, their current status 

  • (married, widowed, divorced, single, living/dead.)              

  • Describe each person.

2.  Describe your relationship with each parent. 
3.  Describe your relationship with each sibling.
4.  How did your parents treat each other?  
5.  How were decisions made in the family?      
  •  Who was dominant? 
6.  How were differences resolved?
7.  How were the roles of husband/wife, mother/father, parent/child portrayed?  
8.  How was love and generosity expressed?
9.  How did the family spend its free time and what forms of entertainment/recreation took place?
10.  What family traditions, practices or qualities would you like to continue?         
11.  What practices, qualities, attitudes or actions would you hope to improve upon?
12.  How did your family relate to the Church?
13.  How do you relate to the Church?
14.  What stands out in memory during the following periods of family life:
  • My Childhood to age 12
  • My Adolescence (ages 13-15)
  • My young adult years (16-19)
15.  How did I do in school and what was my attitude about education?
16.  What kinds of jobs and chores did I have growing up?
  • Inside the family?
  • Outside the family?
17.  How did the family deal with pain and crisis?
18.  What traumas took place within the family?
  • E.g. death, disease, alcoholism, abuse . . . etc 

(Please note that you are not bound to answer this.  If it is too difficult or private you may decline.  It is important to note the impact of such things on married life and that you may wish to discuss them privately with the priest)
